Please review the information below before submitting an online referral or intake form to Center for Child & Family Services (CCFS). Accurate and complete information supports timely review and appropriate service coordination.
CCFS Services
CCFS provides community-based services to support children, youth, adults, and families. Services may include family support and stabilization, adult and youth mental health services, substance abuse services, behavioral and skill-building interventions, care coordination, parenting education, and violence prevention and intervention services. Eligibility and availability vary by program and funding source.
Who May Submit a Referral
Referrals may be submitted by schools, healthcare providers, government agencies, courts, law enforcement, social service professionals, and community partners. If program eligibility is unclear, referrals may still be submitted for review.
Self Referral
Please complete an intake form if you are self referring for you or for a family member.
Review Process
Referrals are reviewed during normal business hours.
CCFS staff may contact the family or referral source for additional information.
Urgent Concerns
This form is not monitored 24/7 and should not be used for emergencies or immediate safety concerns. For urgent situations, contact local emergency services (911) or an appropriate crisis response line (988).
Confidentiality
Information submitted is confidential and shared only with authorized CCFS staff in accordance with applicable federal and state privacy regulations. Please limit sensitive information to what is necessary for referral review.
Consent
By submitting this referral, you confirm that you have authorization to share the information provided. Additional consents will be obtained from the referral source, parent or legal guardian as required.
